Dear Parents,
Students in grades 5, 6, & 7 are preparing for their overnight Wyvern Retreat Program experiences in the next few weeks. To prepare for these trips, there are some school-specific forms that you will need to complete. These are in addition to any forms that you have received for the specific site of the field trip. Here are directions to complete the school forms.
1. School Portal - Field Trip Form
There is a new form on the School Portal that has been assigned to you: 2021-22 Field Trip Form. This form includes a waiver that all parents and students must sign. You will each need to log into the School Portal to sign the form individually. In addition to that, there is a short medical section where you will indicate if the student will be bringing any medications on the trip, and how those should be handled.
To access the School Portal:

Log in with your username and password*
*If you have trouble logging in, click the “Need help logging in?” link on the sign-in page. If, after completing this process, you are still not able to log in, please contact the Front Desk (Betty or Laura) at your Campus.

When you log on to the School Portal you will see a banner stating that you have “Form(s) to Review”. Click on that link, which will take you to the Files and Forms page.

Any incomplete forms will be listed here, as well as the 2021-22 Field Trip Form. Click Continue next to the form name to complete the form. Each parent and student will need to log in to sign the first page of the Field Trip Form. The medical tab only needs to be completed once.
2. SchoolDoc
At the beginning of the school year, you completed medical information about your student on the SchoolDoc platform. Please log back in to this website to review that information and to update the Allergies and Medications form to indicate that your student is attending an overnight field trip.

To access SchoolDoc, go to the Resources tab on the School Portal.

Click on the SchoolDoc logo, which will take you directly to your student’s profile.

Please review all of the information to ensure that nothing has changed.

Pay particular attention to the Allergies and Medications screen. There is a new question at the bottom of this screen that you need to answer regarding this field trip:

If you answer yes to this question, an additional section will appear that will allow you to enter information about any overnight medications that the student will be taking on the trip.
